Prozessunterstützung von adaptiv-evolutionären Informationssystemen in der Medizin (ProMed)

Gegenstand des Projekts "Prozessunterstützung von adaptiv-evolutionären Informationssystemen in der Medizin" (ProMed) ist die Erarbeitung von Architektur- und Prozessbeschreibungsanforderungen an adaptiv evolutionäre Informationssysteme zur Unterstützung institutionsübergreifender Abläufe im Versorgungsnetz (Krankenhäuser, niedergelassene Ärzte, Labore, Kassenärztliche Vereinigungen und Apotheken).
Fachliche Ziele und Motivation: 1) Organisatorische Abstimmung des interinstitutionellen Versorgungsprozesses unter Wahrung medizinisch sinnvoller Reihenfolgen und Bereitstellung von elektronischer Informationsübertragung, 2) Optimierung von Daten- und Prozessqualität und Sicherstellung einer ökonomischen und effizienten Krankenversorgung, 3) Vermeidung von Zeitverlusten wegen Doppeluntersuchungen, 4) Verbesserungen des Versorgungsprozesses (Zeitbedarf der Anamneseerhebung, Reduktion der Komplikationsraten, Terminvereinbarung, etc.). Ziele auf technischer Ebene: 1) Prozessunterstützung durch Datenintegration sowie einer verteilten Ablaufsteuerung als Prozessintegration, 2) Lose Kopplung, 3) minimale Schnittstellen als minimale Anforderungen an beteiligte Systeme. Ziele auf methodischer Ebene: 1) Referenzmodell für eine verteilte Architektur zum Informationsaustausch, 2) Generische Methoden und Werkzeuge zur Prozessmodellierung und für Prozessschablonen, 3) Referenzarchitektur zur Dokument-orientierten Integration von Informationssystemen, 4) Prototyp zum Aufzeigen des Zusammenwirkens von Methoden und Werkzeugen.
Im Rahmen des Projekts wurde eine Analyse der Anforderungen an den Datenaustausch und an die Ablaufsteuerung im Kontext von interinstitutionellen Kooperationen im Gesundheitswesen vorgenommen. Die Strukturen und Informationssysteme in dieser Domäne sind in besonderem Maß gekennzeichnet von evolutionären Veränderungen. Ein Publish/Subscribe System "DEUS", das den traditionellen Pub/Sub-Ansatz um einen dritten Vermittler-Aktor erweitert, wurde entworfen und prototypisch umgesetzt, wodurch ein kooperativer Austausch von elektronischer Patienteninformation in streng dezentralen Systemlandschaften ermöglicht wird. Auf Standard-Konformität zielte die Testumgebung "XdsRig" ab, durch die eine betriebsfähige Open-Source-Umgebung für das IHE XDS Profil bereitgestellt wurde. Ein leichtgewichtiges Prozessmodell zur Unterstützung verteilter kooperativer Abläufe wurde auf Basis des diagnostisch-therapeutischen Zyklus in Form der "DMPS" Plattform erarbeitet. Motiviert aus dem Bereich der Wissensrepräsentation und internationaler Ontologien für das Gesundheitswesen wurde ein natives Open-Source XML-Datenbanksystem um die formale semantische Validierung von OWL-DL Dokumenten mit Hilfe eines Knowledge Reasoners erweitert und wiederum als Open-Source-Projekt "OXDBS" veröffentlicht. Die meisten dieser Vorarbeiten betreffen das Forschungsgebiet der semantischen Systemintegration, die im Gesundheitswesen eine große Rolle spielt.
Als ganzheitlicher und bedarfsorientierter Lösungsansatz wurde in der finalen Projektphase ein dezentrales Fallakten-System namens "alpha-Flow" entwickelt. Jenseits der reinen Probleme einer semantischen Integration fokussiert es sich auf Prozessunterstützung im Sinn einer Beschreibung von kooperativen Arbeitsabläufen sowie dem operativen Einsatz in Form von 'aktiven' Dokumenten. Als benötigte Plattform des reinen Informationsaustauschs wird zukünftig die elektronische Gesundheitskarte (eGK) hoffentlich einen Durchbruch bringen, auf deren Basis die ProMed-Systeme und insbesondere das alpha-Flow-System als Mehrwertdienste eingesetzt werden können. Das alpha-Flow-System für verteilte Fallakten ist wegen seines einzigartigen Dokument-orientierten Prozessparadigmas besonders geeignet für die speziellen Anforderungen in heterogenen und Institutions-übergreifenden Szenarien mit dezentralen Abläufen. Prozesse im Gesundheitswesen sind ebenfalls gekennzeichnet durch ad-hoc Abläufe, mit einer anfänglich unbekannten Menge an Prozessschritten und Teilnehmern. Klassiche Werkzeuge aus dem Bereich Workflow-Management bieten für dezentrale ad-hoc Prozesse unzureichende Unterstützung. Durch eine umfangreiche prototypische Implementierung wurde die Machbarkeit des verteilten Dokument-orientierten Prozessmanagements demonstriert.
